Signature form

I was looking at a thread yesterday and I saw that Wesker's sister needed more info for the signatures. Now I saw someone (I thought it was Jarki) that posted a little info form. I can't find where it was. So the point of this thread is to have either Wesker, or anyone that feels they have necessary info to be given to Wesker for the sigs, to post a general form that needs to be filled out. I would prefer Wesker due to the fact that it is his sister making the signatures.

General picture Wanted: The umbrella Logo, with a bust picture (head and shoulders) of the character 'Buckaroo Banzai' (Peter Weller). Pulling it from this picture is fine:

Sig should be rectangular, (I need to check the dimensions on df, but about half as tall as it is wide shouldn't be a problem).
Transparent background (or the same shade grey as the DF forums)
name in red, title in white, motto in black
All text to the right, name, title, and motto get thier own lines, motto can be fit on a fourth line. Umbrella logo on the left, behind the bust cutout. Bust gets cut off on the bottom of the sig.

Ah. Thank you Jarki. Anyone can use Jarki's info as well or make a good mixture of the two.
